Icon: Fatal 10/31/02: South Padre Island, Texas — On October 23, 2002 a crane hoisting a load failed and dropped the load on a construction worker on a high rise project. We cannot report anything further at this time since there is an ongoing OSHA investigation.

Chennai: Normal work at the Chennai Container Terminal, operated by P&O Ports, was badly affected during the week following a fatal accident on October 13. Senior port officials said operations inside the port are expected to return to normal as of October 25. Due to the accident, a quay crane (QC3) underwent repairs and would be commissioned in a couple of days. The port users, meanwhile, complained that the situation at the terminal was deteriorating ever since the QC3 went out of order following the accident. According to a senior port official, post-accident, all equipment are being re-evaluated for additional safety interlock and mechanical stops.

A construction worker was killed when a crane collapsed on him as he was tying iron rods to its hook, in Damansara on Monday.

Lim Meng Kion, 43, from Labu in Negri Sembilan, suffered severe head injuries during the 5:00 p.m. mishap.

His colleagues escaped unhurt. Police do not suspect foul play.

Lim's remains were sent to the Universiti Malaya Medical Centre for a post mortem.
