The machine involved in the subject accident is a Telsta aerial lift mounted on a Ford truck reportedly owned by Twin Star Cable. Michael Brooks, the driver of the truck, was reportedly standing on the ground with his left arm contacting the truck's cab door when the corner of the platform came into contact with the overhead electrical power line. | 09/19/01: Houston, Texas — Baldwins Industrial Services, Inc. (formerly Phillips Crane & Rigging) in Houston had an incident at Lyondell Citgo Refinery today in the cat cracker unit.
They were assembling a Demag 4800 Crawler. While setting the upper or super structure on the tracks, the Kobelco crane (220 ton capacity) looks as if it got light and lost the load over the side. Ascencion Bermudez, 53, was electrocuted Monday while unloading equipment near power lines at Kelly-Hill Co. in Northmoor. Northmoor police Detective Leroy Williams declined to comment Wednesday about the safety standards in place when Bermudez died. Bermudez's son, Andres Bermudez, said he was glad police and OSHA were investigating. “We just want to make it safer for the rest of the workers so it won't happen again,” he said. “I still have a brother-in-law working there.” Williams said Bermudez, of Kansas City, Kan., died while helping unload a railroad tie end server from a flatbed truck. The man was touching the server with his right hand as it was being carried by a crane. He was electrocuted when the crane came in contact with an overhead power line, sending more than 12,000 volts of electricity through his body. The crane operator was uninjured. Bermudez had worked for Kelly-Hill for about 25 years. He recently started his own trucking company, family members said. He is survived by his wife, Gloria, and their four children and three grandchildren. His son said the couple were looking forward to the birth of a fourth grandchild and to their 30th wedding anniversary in December. “He was a good-natured, caring person with really strong family ties,” Andres Bermudez said. This is the 24th (AVOIDABLE!) incident reported to this site this year in which power lines were involved. Again, all were avoidable. Just don't work cranes near energized power lines. |
The two workmen died early Monday after the cherry picker they were working in was hit by a section of crane cable that had snapped. Lt. July Rivers, spokesman for the Osceola Sheriff's Office, later identified the victims as: David Knight, 32, of Lacoochee in Pasco County, and Enrique Aguilar, 22, of Mount Dora in Lake County. The Florida Highway Patrol and Bay County Sheriff's Office said a crane operated by Robert Waters, 40, of Panama City, struck power lines beside the bridge. No one was injured, and property damage was minimal. Granite Construction, the Tampa-based firm contracted to build the new bridge. This is the 23rd (AVOIDABLE!) incident reported to this site this year in which power lines were involved. Again, all were avoidable. Just don't work equipment near energized power lines. |
