Summer of 1995: North Carolina — Crane accident in the Research Triangle Park

The accident in the attached photo happened in the Research Triangle Park in North Carolina in the summer of 1995. The investigation determined three causes of the accident.

1. Operator error: Booming up letting the limit switch (which failed) stop the boom.

2. Failure to inspect and maintain the limit switch.

3. Failure of the root welds for the forestay cable clips. There were no injuries. The operator declined an opportunity to take a drug test got in his truck and left.

A construction worker from Radlett plunged to his death after an unstable crane toppled over, a court heard on Friday.

Anthony Fear, 38, was thrown five storeys to the ground when the work platform he was dismantling came away from the building.

Galen V. Johnson, Jr., a 42-year-old employee of Louisiana Crane and Machinery, was working as a sub-contractor in a roadway near a UOP manufacturing unit. Caddo Parish Sheriff's deputies report that at about 10:30 a.m. Johnson was assembling a crane before beginning work at the plant site when the accident occurred.
Note: In 1995 there weren't many crane accident related reports on the Internet.
